On February 17th UAE CGAA investigators released a preliminary report regarding an Emirates 777-300 takeoff on December 19th.
The Takeoff resulted in the 777 remaining at very low altitude after take off.

The report is hinting at one of the theories involving the MCP altitude selector being set to 0000 and selecting 4000 feet only after the Flight director was switched on resulting in ALT mode being the active mode in lieu of TO/GA mode.
